1. Do you have a new fever, chills, sore throat or shakes?
2. Do you have a new/worsening cough and shortness of breath?
3. Have you travelled outside of Canada within the last 14 days?
4. Have you had contact with a sick person who has travelled outside of Canada in the last 14 days or has or may have COVID-19?

If any of the responses to the above questions are YES then we are unable service you due to health concerns of fellow Ontarians, staff, and physicians.

We respectfully ask that you maintain hygiene and call Public Health, refer to online resources including Public Health or the MOH, or contact your physician for further information.
