What To Bring
Please bring the following to your appointment at any Merivale Medical Imaging location.
Health Card
Required for all OHIP-covered services.
Requisition
From your referring physician or healthcare provider.
Payment Info
Due at time of service for uninsured or Quebec patients. We accept Debit and Credit Card.
Prior Imaging
Previous imaging studies or reports, if available.
Cancellations & Missed Appointments
If you are unable to attend your appointment, please notify us at least 24 hours in advance. Failure to cancel with sufficient notice may result in a missed appointment fee.
To cancel or reschedule: call 613-727-1072 option “1”, visit our booking office, leave a voicemail (accepted 24 hours a day), or contact us via live chat. Please include your phone number and preferred callback time.
Payment Methods & E-Referrals
For Quebec residents and patients without provincial health insurance coverage, payment in full is due at the time services are rendered.

Referring physicians can submit requisitions directly through Ocean eReferral.

Merivale Medical Imaging accepts requisitions from all licensed physicians. Our editable PDF requisition can be downloaded, completed, and sent with your patient before their appointment. Requisitions are accepted at any licensed healthcare facility, including hospitals and independent health facilities (IHFs).
